Hypothyroidism: An underactive thyroid can lead to increased levels of thyrotropin-releasing hormone (TRH), which stimulates prolactin production.

Chronic Kidney Disease: Impaired kidney function can result in decreased clearance of prolactin, leading to elevated levels in the bloodstream.

Certain Medications: Some medications, particularly antipsychotics and antidepressants, can interfere with dopamine, which usually inhibits prolactin secretion.

How Elevated Prolactin Levels Affect Fertility

Elevated prolactin levels can significantly impact fertility in both men and women. Understanding the effects of hyperprolactinemia is crucial for those struggling with infertility.

Effects on Women

In women, high prolactin levels can disrupt the normal menstrual cycle. This disruption may manifest as irregular periods or amenorrhea (absence of menstruation). Elevated prolactin can inhibit the release of gonadotropin-releasing hormone (GnRH), which is essential for the production of follicle-stimulating hormone (FSH) and luteinizing hormone (LH).

Without adequate levels of FSH and LH, the ovaries may not function properly. This can lead to difficulties with ovulation, making conception challenging. Women with hyperprolactinemia may experience the following symptoms:

  • Irregular menstrual cycles
  • Ovulation issues
  • Galactorrhea (unexpected milk production)
  • Decreased libido

In men, elevated prolactin levels can lead to decreased testosterone production. Low testosterone can cause several issues related to fertility, including:

In cases where a prolactinoma does not respond to medication or if there are significant side effects, surgery may be considered. Surgical intervention aims to remove the tumor and alleviate the excess prolactin production.
